What We Provide

Case Management

Our dedicated case manager serves as the initial point of contact for all program participants. Working closely with each individual, they develop personalized reentry plans and provide warm introductions to partner organizations. The case manager coordinates services chronologically to create the best opportunity for stability and success.

Personalized plans, not cold referrals

Housing Assistance

We work to secure stable housing for participants within the first two weeks of release. Through partnerships with the Salvation Army, Jimmie Hale Mission, Three Hots and a Cot, Oxford House, and the Housing Authority of Greater Birmingham, we connect individuals with temporary and permanent housing solutions.

Healthcare Access

Through Alabama Regional Medical Services (ARMS) and other partners, participants receive comprehensive healthcare including medical care, mental health assessments, and counseling services. Every participant is offered an initial mental health assessment within the first month of enrollment.

Substance Use Treatment & Recovery

All participants with a substance use history receive an ASAM assessment to determine the appropriate level of care. Treatment options include inpatient rehabilitation through Aletheia House, Medication Assisted Treatment (MAT), peer support from certified recovery specialists, and ongoing aftercare programs. Narcan and fentanyl test strips are provided to all clients.

Employment & Workforce Development

Hope Inspired Ministries and the Salvation Army provide job training, workforce development skills, and connections to employers. Services include soft skills training, financial management, resume building, and attendance at Second Chance hiring fairs. Programs at Jefferson State Community College and STRIVE are also available.

From part-time to full-time careers

Peer Support

The Offender Alumni Association, led by formerly incarcerated individuals, provides emotional and social support, life skills training, and mentorship. As peers who understand the journey firsthand, they are uniquely positioned to help participants navigate the challenges of reentry.

Support from people who've been there

Identification & Benefits

Community on the Rise helps participants obtain essential documents including birth certificates, social security cards, and state IDs. They also assist with applications for government benefits such as food stamps, social security, and Medicaid or Medicare.

Essential documents secured quickly

Transportation

Transportation assistance ensures participants can access all services, attend appointments, and get to work. Removing transportation barriers is critical to successful reentry.
